Bienvivir All-Inclusive Senior Health (“Bienvivir”) is a community-based, patient-centered, comprehensive health care delivery system that advocates and promotes quality of life, optimum independence, dignity, and choices in a nurturing environment for frail seniors. Since 1987, Bienvivir has served the frail seniors of El Paso, Texas through the provision of the Program of All-Inclusive Care for the Elderly (“PACE”).

PACE is a unique managed care benefit for frail seniors (referred to as participants) age 55 and older who are certified by the state as needing nursing home level care and who reside in a PACE service area. PACE programs coordinate and provide comprehensive medical and support services so that participants can remain independent and stay in their homes for as long as safely possible.

Bienvivir is currently accepting applications for the following position:

 

LVN CONSULTATION NURSE

The Consultation Nurse works under the direct supervision of the Clinic Manager in the daily operation of the clinic by answering and gathering data on all incoming phone calls to the clinic and by generating specialist consults orders for the participants as they are approved by the Primary Care Provider (PCP). The Consultation Nurse is responsible for ensuring the accuracy of the consult orders.

RESPONSIBILITIES

1. Answers and documents on all incoming telephone calls to the clinic. Obtains information necessary to provide information regarding participant’s health status or concerns to the Participant Manager and/or PCP.

2. Processes and tracks all specialist consult orders generated by the PCP. 

3. Receives all returned consultation paperwork for specialist appointments to ensure they are complete and reviews the documents with the PCP for approval.

4. Transcribes approved specialist recommendations into the EMR to create new consult orders and routes to the PCP for co signature.

5. Reviews and coordinates all consults with the Participant Manger and all other necessary staff involved to include pre-procedure care such as medications, labs, and other information needed prior to a procedure.

6. Audit consults orders on a daily basis to ensure accuracy and correct all missing information on consults returned by the Appointments Schedulers.

7. Communicates directly with specialist offices to clarify recommendations and obtain prep instructions for pending procedures or treatments.

8. Review daily laboratory results researches high, low results and medications before presenting them to the PCP.

9. Reviews daily x-rays results and provides results to Participant Managers.

10. Conducts monthly medication reviews for assigned participants.

11. Provides coverage for Medication and Treatment nurses.

12. Performs other duties as assigned by the Clinic Manager.

 QUALIFICATIONS / REQUIREMENTS:

1.      A graduate of an accredited nursing program with a license to practice in the state of Texas as a vocational nurse.

2.      One year of experience giving medications, preferable with the geriatric population

3.    Bilingual (English/Spanish) required

4.     Possess strong computer skills
