Community Support Services Provider
Description:
We are seeking a motivated and compassionate individual to join our team as a Community Support Services Provider (CSSP). This role provides hands-on, community-based behavioral health services to individuals experiencing mental health challenges, behavioral needs, and/or functional impairments. Services are delivered under the supervision of a Licensed Mental Health Professional (LMHP) in accordance with Texas Medicaid and Health and Human Services Commission (HHSC) requirements.
The CSSP supports individuals and families by promoting independence, stability, and recovery through skills training and case management services.
Key Responsibilities:
- Provide Skills Training and Case Management services under the supervision of an LMHP
- Assist individuals in developing daily living skills, coping strategies, and social functioning
- Coordinate and link clients to community resources such as housing, employment, medical care, and social services
- Advocate for clients to ensure access to appropriate services and supports
- Monitor client progress and communicate updates to the supervising clinician
- Maintain accurate, timely, and compliant Texas Medicaid documentation
- Support and educate families and natural supports on effective strategies for managing behaviors and improving functioning
- Deliver services in home and community settings, as required by the treatment plan
Requirements:
- Meets Texas Medicaid / HHSC requirements for a Community Support Services Provider
- High school diploma or equivalent required
- Completion of required agency and state-mandated training
- Ability and willingness to travel for in-person, community-based services
- Strong verbal and written communication skills
- Ability to work independently while collaborating with a multidisciplinary team
- Commitment to ethical practice, professional development, and person-centered care
Preferred Qualifications:
- Experience working with individuals with mental health, behavioral, or developmental needs
- Prior experience providing skills training, case management, or community-based services
- Knowledge of behavioral health practices and child, adult, and family support strategies
- Associate’s or Bachelor’s degree in Human Services, Psychology, Social Work, or a related field (preferred but not required)
