Case Manager – Plano Homelessness Prevention Program (HPP)

Location: Plano, Texas

Position Type: Part TIme (25-30 hours)

Position Summary

The Plano Homelessness Prevention Program (HPP) Case Manager provides comprehensive case management services to households at risk of eviction within the City of Plano. This role focuses on preventing homelessness through rent assistance, stabilization planning, and coordinated supportive services while ensuring compliance with program and funding guidelines.

Key Responsibilities

• Conduct client intakes and eligibility screenings
• Develop individualized stabilization plans
• Coordinate rent assistance payments
• Maintain accurate client documentation and compliance
• Collaborate with internal staff and partner agencies
• Track outcomes and assist with reporting

Qualifications

• Associate’s or Bachelor’s degree in Social Work, Human Services, or related field
• 1–2 years of case management experience
• Knowledge of homelessness prevention or housing programs
• Strong organizational and communication skills
